Below is my letter to the editor of The New England Journal of Medicine that they published about this study: Protein, too, doesnt look so evil when one considers the 2010 trial published in The New England Journal of Medicine that found individuals who had recently lost weight were more likely to keep it off if they ate more protein, along with the 2005 OmniHeart trial that reported individuals who substituted either protein or monounsaturated fat for some of their carbohydrates reduced their cardiovascular risk factors compared with individuals who did not. The rivalry between Atkins and Ornish was related to food intake. Prev Cardiol. A randomized trial of a low-carbohydrate diet for obesity. Total fat and animal fat intake were higher and carbohydrate intake was lower in those with recently diagnosed diabetes or previously undiagnosed diabetes in the multinational, multicenter study of the Mediterranean Group for the Study of Diabetes (17). But Ornishs arguments against protein and fat are weak, simplistic and, in a way, irrelevant. Also, as I wrote in my oped in The New York Times, my colleagues and I have conducted randomized controlled trials that these same diet and lifestyle changes reverse the progression of other common chronic diseases. Stevens A, Robinson DP, Turpin J, et al. Wing RR, Vazquez JA, Ryan CM. (Also: the heavy protein consumers in the study were consuming nearly 30 percent more protein than the average American does.) Judging the quality of a study by the number of patients is like judging the quality of a book by the number of pages. When you eat less fat, you consume fewer calories without having to eat less food, thereby increasing satiety without adding calories. It found that individuals assigned to eat high-fat (41 percent calories from fat), Mediterranean-style diets for nearly five years were about 30 percent less likely to experience serious heart-related problems compared with individuals who were told to avoid fat. In fact, per capita consumption of fat has risen by 10 Ib/year since 1975, whereas per capita consumption of simple carbohydrates has increased even more, by 20 Ib/year (5). But heres the thing: The patients who followed his diet also quit smoking, started exercising and attended stress management training. Dietschy JM, Brown MS. It sounds good, but it is not true. The Atkins diet caused the most coronary artery blockages whereas a diet low in fat and high in unrefined carbohydrates caused the least amount of blockages. 1998;339:12-20. Eat more meat, butter and eggs, theyre good for you (prominently pictured on the cover of their books), all those experts have been wrong. The impact of protein intake on renal function decline in women with normal renal function or mild renal insufficiency. The decrease in the percentage of calories from fat during the period 1971 to 1991 is attributed to an increase in total calories consumed; absolute fat intake in grams actually increased. This is why I use the USDA data (which tracks consumption of the entire food supply, not just a tiny sample). Insulin may also raise the secretion of lipoprotein lipase, increasing the uptake of fat into cells, leading to weight gain (10). And although Ornish is right that I lack clinical experience, when analyzing evidence, distance can be useful. The March 2014 study in Cell Metabolism did distinguish between animal protein and plant-based proteins. Could it be that our attempts to reduce fat have in fact been part of the problem? Despite being told to eat less fat, he says, Americans have been doing the opposite: They have actually consumed 67 percent more added fat, 39 percent more sugar and 41 percent more meat in 2000 than they had in 1950 and 24.5 percent more calories than they had in 1970. Yes, Americans have been eating more fat, sugar and meat, but we have also been eating more vegetables and fruits (pdf)because we have been eating more of everything. N Engl J Med. Potential benefits and risks have not been tested adequately. In our debates, Dr Atkins often claimed that his diet can reverse coronary heart disease (CHD) but never published any peer-reviewed data to support this assertion, nor has anyone else, including advocates of similar diets such as the Zone and the South Beach diets (34). When Moyer accuses me of having inadequate scientific evidence to support my statements, I must respectfully and strongly disagree. A 2014 meta-analysis similarly reported much higher mortality risks associated with processed meat compared with red meat consumption and found no problems associated with white meat. After multivariate adjustment for major lifestyle and dietary risk factors, the pooled hazard ratio (HR) (95 percent CI) of total mortality for a one-serving-per-day increase was 1.13 (1.071.20) for unprocessed red meat and 1.20 (1.151.24) for processed red meat. They concluded, A low-carbohydrate diet based on animal sources was associated with higher all-cause mortality in both men and women whereas a vegetable-based low-carbohydrate diet was associated with lower all-cause and cardiovascular disease mortality rates. Another major research article studied 43,396 Swedish women over 15 years. 2001;73:1010-1018. In assessing the health effects of different diets its important to measure the disease process itself, not just risk factors such as blood pressure and cholesterol levels. The goal is to lose weight in ways that enhance health rather than in ways that may harm it. Samaha FF, Iqbal N, Seshadri P. A low-carbohydrate diet as compared with a low-fat diet in severe obesity. The editor of JAMA Internal Medicine invited me to write an accompanying editorial for this study. The case for a low-fat, highcarbohydrate diet. In educating patients in an evidenced-based practice, it may be helpful to summarize an optimal diet as one that is high in good carbohydrates (complex carbohydrates), good fats (n-3 fatty acids) (62), and good protein (plant based) and low in ones that are less healthful. For example, an important article, published in The New England Journal of Medicine, reviewed data showing that high-protein, low-carbohydrate diets promote coronary artery disease independent of their effects on traditional risk factors such as blood pressure and cholesterol levels. Its possible that quitting smoking, exercising and stress management, without the dieting, would have had the same effectbut we dont know; its also possible that his diet alone would not reverse heart disease symptoms. Taubes G. What if its all been a big fat lie? HDL is important only to the extent that it affects atherosclerosis and myocardial perfusion, it is not a disease. Plus, discouraging the intake of entire macronutrient groups can backfire. In rural China, for example, the average LDL is less than 95 mg/dL. Reductions in total cholesterol, LDL-C, triglycerides, and total-to-HDL cholesterol ratios were significant only in patients who were following either a 10% fat diet or a 15% fat, calorie-controlled diet. In one of these studies, low-density lipoprotein cholesterol (LDL-C) increased from 118 to 121 mg/dL on an AHA/NCEP low-fat diet and increased from 114 to 118 mg/dL on a high-protein/low-carbohydrate diet (19). Im not cherry-picking data; Im looking at the preponderance of evidence from many studies by leading investigators such as those at Harvard School of Public Health. The title is confusing and potentially harmful to many readers. Urinary excretions of calcium and acids are correlated positively with intakes of animal and nondairy animal protein but are correlated negatively with plant-protein intake (53). Mean 12-month weight loss was as follows: Atkins, -4.7 kg (95% confidence interval [CI], -6.3 to -3.1 kg), Zone, -1.6 kg (95% CI, -2.8 to -0.4 kg), LEARN, -2.6 kg (-3.8 to -1.3 kg), and Ornish, -2.2 kg (-3.6 to -0.8 kg). MAIN OUTCOME MEASURES: There are no data showing that the physiologic reduction of HDL-C levels with a low-fat diet is detrimental, especially in that LDL-C usually decreases more than HDL-C (42). In addition, patients on an Atkins diet are counseled to take n-3 fatty acids in the form of fish oil (something else we agreed on), which is known to reduce triglycerides significantly. Brody J. An Atkins diet often shows a greater reduction in triglycerides by comparison. In a 2007 clinical trial led by Gardner researchers randomly assigned 311 individuals to four groups: One group was assigned the high-fat, high-protein and low-carbohydrate Atkins diet; the second was assigned Ornishs very low-fat vegetarian diet, which requires consuming fewer than 10 percent of calories from fat; the third was assigned the Zone diet, which aims for a 40/30/30 percent distribution of carbohydrate, protein and fat; and the fourth was assigned the high-carbohydrate, lowsaturated fat LEARN (for: lifestyle, exercise, attitudes, relationships, nutrition) diet.
